FLHealth CHARTS State and County Profiles

To enhance local awareness about weight, the Florida Department of Health has a comprehensive healthiest weight profile for the state and each of the 67 counties. FLHealth CHARTS is designed to support community health assessments, provide data for health and weight related research and encourage communities to continue their efforts to improve nutrition and increase physical activity. Other Resources

Healthiest Weight Florida encourages health throughout the life span. There are many useful national, statewide, and local resources available for programs and projects to promote healthy weight.

Florida Breastfeeding Coalition
A statewide multidisciplinary group of individuals, businesses and organizations with the mission to improve the health of Floridians by working collaboratively to protect, promote and support breastfeeding.

Baby-Friendly USA (BFUSA)
The nonprofit national authority for the World Health Organization and the United Nations Children’s Fund (UNICEF) Baby-Friendly Hospital Initiative (BFHI). The mission of BFUSA is to assess, accredit and designate birthing facilities that meet the BFHI criteria for implementing the Ten Steps to Successful Breastfeeding and follow the International Code of Marketing of Breast-milk Substitutes -- providing mothers and babies with the early support needed to achieve successful breastfeeding, an essential foundation for a healthy nation.

Healthy Kids, Healthy Future
Healthy Kids, Healthy Future continues the work of Let’s Move! Child Care and encourages and supports child care and early education providers to make positive changes in their programs in order to work toward a healthier future for children. This initiative is voluntary and for all types of programs: family child care homes, centers, early head start and head start programs, preschool, tribal programs and faith-based programs. Healthy Kids, Healthy Future offers childhood obesity prevention resources and tools to assist child care and preschool providers.

National Resource Center for Health and Safety in Child Care and Early Education
The new set of national standards describing evidence-based best practices in nutrition, physical activity and screen time for early care and education programs. These updated standards are part of the new comprehensive Caring for Our Children: National Health and Safety Performance Standard Guidelines for Early Care and Education Programs.

Shape America
One of the top priorities for Head Start Body Start National Center for Physical Development and Outdoor Play is to inform the Office of Head Start in setting national priorities and developing policies related to active play, physical activity, high quality outdoor play spaces and learning environments, and proper nutrition.

Let's Go
A nationally recognized childhood obesity prevention program implemented throughout Maine and in a few communities in neighboring states. They partner with schools, child care and out-of-school programs, healthcare practices and community organizations to change environments where children and families live, learn work and play.

Healthy District Award
The tool is first and foremost a self-assessment and planning tool that will help school districts determine where they are currently and what they might be able to do to remove health related barriers to learning and work toward recognition as a Florida Healthy District. It may take a few years to reach the level of recognition but the tool will help identify where a school district is currently and what it might be able to work on.

Comprehensive School Physical Activity Program (CSPAP)
A multi-component approach by which school districts and schools use all opportunities for students to be physically active, meet the nationally-recommended 60 minutes of physical activity each day and develop the knowledge, skills, and confidence to be physically active for a lifetime. A CSPAP reflects strong coordination and synergy across all of the components: quality physical education as the foundation, physical activity before, during, and after school, staff involvement, and family and community engagement.

Wellness Council of America (WELCOA)
One of the nation’s most respected resources for building high-performing, healthy workplaces. With a 30 year history and more than 5,000 corporate members, WELCOA has an impeccable reputation for helping business and health professionals improve employee well-being and create healthier organizational cultures.

National Wellness Institute (NWI)
It was formed to realize the mission of providing health promotion and wellness professionals unparalleled resources and services that fuel professional and personal growth.

Employee Wellness
A workplace health program is a health promotion activity or organization-wide policy designed to support healthy behaviors and improve health outcomes while at work. These programs consist of activities such as health education and coaching, weight management programs, medical screenings, on-site fitness programs and more.

Heart Health +
Heart disease has been the leading cause of death in Florida and the U.S. for over 70 years, which means it has affected a countless number of Floridians. Heart Health+ aims to prevent heart disease in the communities that need it the most. Launched November 2016, this pilot program implements evidence-based strategies to affect faith based organizations, health systems and local communities.

Diabetes Prevention Program (DPP)
People at risk for type 2 diabetes can prevent the disease by increasing physical activity and improving nutrition. The National Diabetes Prevention Program is shown to reduce risk by 58% among participants. Click to take a risk test and find a DPP near you.

American Diabetes Association (ADA)
Leading the fight against the deadly consequences of diabetes and fight for those affected by diabetes by funding research to prevent, cure and manage diabetes. The ADA delivers services to hundreds of communities and provides objective and credible information.

Supplemental Nutrition Assistance Program (SNAP)
The Supplement Nutrition Assistance Program offers nutrition assistance to millions of eligible, low-income individuals and families and provides economic benefits to communities.

Fresh Access Bucks (FAB)
The Fresh Access Bucks Program makes fresh, local produce more affordable and accessible to low income families while supporting Florida’s farmers and enhancing local economies. It does so by increasing the purchasing power of Supplemental Nutrition Assistance Program (SNAP) participants by providing a one-to-one match for Florida grown fruits and vegetables. A SNAP cardholder who spends $40 of their SNAP benefits at a participating market receives an additional $40 to purchase fresh, local produce.

Women, Infants and Children (WIC)
The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides Federal grants to States for supplemental foods, health care referrals, and nutrition education for low-income pregnant, breastfeeding, and non-breastfeeding postpartum women, and to infants and children up to age five who are found to be at nutritional risk.

Farmers' Market Nutrition Program (FMNP)
The Farmers’ Market Nutrition Program (FMNP) provides WIC participants with coupons that can be used to purchase locally grown Florida fruits and vegetables. This produce can be purchased at authorized farmers’ markets from April 1 to October 8.

Supplemental Nutrition Assistance Program Toolkit
A toolkit that is an Obesity Prevention Toolkit for all States. It features evidence based policies, systems and environmental changes that support direct education and social marketing and ways to evaluate them across various settings.

My Plate offers ideas and tips to help you create a healthier eating style that meets your individual needs and improves your health.

Check Out Healthy
A toolkit designed for leaders of local coalitions, communities, and public health that focuses on enhancing access and availability of healthier options in food stores. It includes evidence-based methods and real life stories as examples to implement strategies in communities.

Order Up Healthy
A toolkit designed for leaders of local coalitions, communities, and public health that focuses on enhancing access and availability of healthier options in restaurants. It includes evidence-based methods and real life stories as examples to implement strategies in food environments of restaurants.

Healthy Food Access Portal
A portal that contains healthy food access resources, tools to help find money for healthy food access projects and programs, and resources for healthy food access policy and retail strategy.

Active Design Resources
A collection of resources to assist professionals in design, policymaking, and real estate as well as community advocates to translate health-related design research to architecture and urban planning practice in order to improve health.

A Guide to Building Healthy Streets
This guide can help communities ensure their Complete Streets policy becomes more than words on paper and creates real, on-the-ground change. It focuses on how public health practitioners, in particular, can collaborate with other agencies to implement Complete Streets. State and local public health professionals have many opportunities to shape how decisions about street design are made, and they play an important role in supporting and leading efforts to create healthier streets.

Transportation and Health Tool
The Transportation and Health Tool (THT) was developed by the U.S. Department of Transportation and the Centers for Disease Control and Prevention to provide easy access to data that practitioners can use to examine the health impacts of transportation systems. The tool provides data on a set of transportation and public health indicators for each U.S. state and metropolitan area that describe how the transportation environment affects safety, active transportation, air quality and connectivity to destinations.

Bicycle and Pedestrian Program
The Federal Highway Administration's Bicycle and Pedestrian Program promotes safe, comfortable, and convenient walking and bicycling for people of all ages and abilities. They support pedestrian and bicycle transportation through funding, policy guidance, program management and resource development.

Built Environment and Health Initiative
An initiative (also known as the Healthy Community Design Initiative) gives state and local governments the knowledge and tools needed to make planning and transportation decisions that protect American lives and reduce healthcare costs. The Center for Disease Control and Prevention’s Built Environment and Health Initiative is the only federal program whose primary purpose is to improve the health of all Americans through evidence-based changes to the built environment.

Healthy Community Design Checklist
A toolkit for leaders in planning, public health, and the general public to help incorporate health in community planning. It includes a checklist that educates residents on healthy design, covering topics such as active living, food choices, transportation choices, public safety, social cohesion, social equity, and environmental health.

Healthy Community Planning Toolbox
A toolbox that helps community leaders create healthier communities through planning process tools that use a health lens to plan effectively and through policy intervention tools that affect the built environment.

A Practitioner’s Guide for Advancing Health Equity: Community Strategies for Preventing Chronic Disease
A resource designed to help professionals in public health, design, policymaking, real estate and the community include equity in practice. The guide contains tobacco-free living, healthy food and beverage, and active living strategies in addition to foundational skills.

Tools for Putting Social Determinants of Health into Action
Conditions in the places where people live, learn, work and play affect a wide range of health risks and outcomes. These conditions are known as social determinants of health (SDOH). This website provides CDC resources for SDOH data, tools for action, programs and policy. They may be used by people in public health, community organizations, and health care systems to assess SDOH and improve community well-being.
